My Survey MySQL is a perl driven module that utilizes the features of MySQL server for survey manipulation. You can create a table to allow your users to feed their datas in the text bar and can store them with the backend database which can be retrieved immediately for viewing, editing and other purpose. HTML tables can be used to display the results also, graphical evaluated graph charts can show the survey results. Exporting the result into spreadsheet is available. Live demo simply explain the features.
